Forming Your Team
The following is derived from pages 2-3 of the Team Application Instructions.
Team Leader
Each team must have a team leader who will convene the team and maintain the team’s momentum during the duration of the project. The team lead will be a staff member of the applicant Qualified Organization. (See the Challenge Overview for more information about eligible applicants.) A co-lead from another member organization is also encouraged.
Team Members
NCMM is looking for each applicant to have a core team of no less than 5 team members. NCMM will select teams whose members
- Each bring a different perspective and knowledge base as it applies to the problem area
- Have broad and diverse networks
- Are committed to remaining engaged in solving the chosen complex mobility problem
- Are invested in learning about existing factors affecting access to healthcare destinations, including through conversations with potential customers and other stakeholders
- Are open to new ideas and problem-solving approaches and comfortable working on a project in which a clear solution is not immediately visible and in which the process getting to a solution will be the result of continual stages of iteration
Required Five Team Members
Each team must have at least five members, as specified below. Note: The team leader is one of the five members.
- 1 or more people representing a patient perspective (e.g., actual patient, patient advocacy group, caregiver)
- 2 or more mobility experts (e.g., public or private transportation provider [of bus, van, vanpool, carpool, taxi, bike-sharing services, etc.], transportation management association, volunteer transportation network, ridesharing coordinator, mobility manager, transportation planner)
- 1 or more local healthcare providers relevant to the team’s problem area (e.g., hospital, community-based medical center, treatment center, rehabilitation center). For example, applicants proposing to improve mobility access for dialysis patients must include a representative from a local dialysis center. Applicants proposing to improve mobility access for behavioral health patients must include a representative from a behavioral health center.
- 1 or more representatives from a public, private, or nonprofit social services organization who are providing case management services or other service
Additional team members
Teams may also include up to an additional five members, drawn from the following:
- Ancillary healthcare providers (e.g., physical therapists, community-based care transition programs, home-health aides)
- Community-based organizations providing services to older adults, people with disabilities, or people with limited income
- Local healthy living advocates and practitioners
- Local elected or public agency officials
- City or county health department
- Health plans, insurance providers, managed care providers, accountable care organizations
- Naturally occurring retirement community (NORC)
- Technology sector
- Others who would round out team experience, networks, and skills
